Job Title:                     Early ACCESS Service Coordinator

 

Responsible to:            Designated Regional Grantee/Signatory Partner Supervisor

 

 

Job Description:           The Early ACCESS Service Coordinator works collaboratively with families and service providers to identify families’ strengths, concerns, and priorities through the development and implementation of an IFSP

Qualifications:

·         Knowledge of IDEA Part C, the system of payments for services in Iowa and other pertinent information

·         Knowledge of community resources and services

·         Ability to facilitate communication, both written and verbal, among team members

·         Ability to organize work and complete tasks in a timely manner

·         Understands and uses family centered practices

·         Ability to relate easily to people

·         Understands and agrees to abide by confidentiality standards

·         Is available year round

·         Passes required child abuse and criminal records check

 

Tasks:

·         Completes Early ACCESS Service Coordinator Competency training

·         Attends Early ACCESS Service Coordinator and related meetings

·         Provides regular face-to-face contact with families

·         Coordinates services across agencies

·         Serves as single point of contact in helping parents to obtain the services and assistance they need

·         Informs families about resources and services in their community

·         Identifies, coordinates, and arranges necessary evaluations and assessments

·         Facilitates access to community resources and services that could help the child and family

·         Helps families organize and prepare to develop their Individualized Family Service Plan (IFSP)

·         Writes, reviews, and updates the IFSP

·         Collaborates with all partners in order to meet the needs of the family

·         Assures that services families receive are delivered in a smooth and timely way

·         Informs families about advocacy organizations and support groups

·         Facilitates development of a transition plan prior to the child exiting Part C

·         Provides timely accurate documentation of Service Coordination activities

·         Manages case file, including the IFSP and all necessary related information and reports

·         Provides necessary information at formal periodic reviews and annual evaluations of the IFSP
