Early ACCESS Service Coordination Skills Assessment Guidance

 

 

To assess Service Coordination competencies use the following tools:
 

1.      Review the Service Coordinator Job Duties

2.      Review the Early ACCESS Service Coordinator Competencies

·         Each SC Competency Based Training Module is focused upon one of the competencies, e.g. Module 1 Federal, State and Local Rules, Regulations, and Procedures for the Early ACCESS System, covers the first SC competency.

3.      Review the Rubric Profile of Family Centered Service Coordination (SC) for Service Coordinators

·         Best Practice, Compliance, and Non-Compliance behaviors are listed

4.      Complete the Service Coordinator Competency Assessment

·         There is a separate section for each competency

5.      Complete the Service Coordinator Competency Verification form.

 

Step 1.            Record the demonstrated level of competence in each of the areas listed on the Service Coordinator Competency Assessment     

NOTE:              Complete one assessment for each Service Coordinator

                       

Step 2.            Provide each of your Service Coordinators with all of the tools and the directions listed above, and a copy of this sheet so that all SCs may complete their individual Self Assessments.

 

Step 3.            Determine the competency areas in which skills are not demonstrated within the Compliance or Best Practices categories and indicate areas in which the SC would benefit from additional training and support. 

 

Step 4.            Confer with SCs to discuss which training s/he will take.

 

Step 5.            For those who have demonstrated Best Practice and/or Compliance levels of performance, note this on the Service Coordinator Competency Verification form.

 

Step 6.            FAX a copy of the Service Coordinator Competency Verification form to Joan Twedt at 515-242-6019.  Once a Service Coordinator has completed the SC Module training or been acknowledged by her/his administrator as fully competent in all five competency areas, a certificate will be awarded.

(Winter 2005 – November, December, January CD Rom
 available from On Site Training Coordinators)

 

2.     Module 3            The Early ACCESS Process           

(Winter 2006 - February, March, April delivered once in
 each quadrant of the state by State Staff)

 

3.     Module 5            Infant and Toddler Development

(Summer 2006 - May, June, July, delivered once in each
 quadrant of the state by State Staff)

 

                        4.     Module 2            Family Centered Practices

(Spring 2006 - February, March, April delivered by
Approved Trainers by quadrants in the state)

 

                        5.     Module 4            Coordinating Community Resources

(Summer 2006 – May, June, July delivered by
Approved Trainers by quadrants in the state)
