1. Before referral to vision staff
Obtain the current optometrist/ophthalmologist eye report.
Have parents sign a "Release of Information" to release the eye report to the AEA.
Contact the vision staff at the AEA regional office to determine if appropriate referral.
If appropriate referral, referring person should complete and send "Request for Service" form, eye report and signed release to vision staff at the appropriate AEA regional office.
2. Referral for Evaluation/Assessment
Once referral is received, the Teacher of the Visually Impaired and the Orientation and Mobility Specialist will jointly develop questions for the "Intent and Consent" with the referring team.
3. Evaluation/Assessment
Functional Vision Assessment is completed.
Orientation and Mobility Assessment is completed.
Reports are written.
Staffing to determine eligibility for services and development of Individualized Education Program (IEP/IFSP).
